Preschool Evaluation Reimbursement STAC-5
Under Section 4410 of the New York State Education Law, the CPSE responsible district may file a Request for Reimbursement for students who have been referred to the CPSE Committee for preschool evaluations.
Districts complete the Preschool STAC-5's, obtain proper district signature, and submit the STAC-5 to the appropriate municipality for signature. The municipality then electronically submits the STAC-5 information to the STAC and Medicaid Unit.
